Facility Requests

The St. Mark Facility Request Form is to be used to request space for an event that is outside of a ministry, group, or organization's normal meeting time.

Carefully read and complete all form sections relevant to your request.

• Facility requests are encouraged to be submitted as far in advance as possible, but no later than 10 business days before the event.
• Requests for use of facilities for large-scale events should be submitted 3-6 months in advance.
• Approval process times may vary.
• Users will receive an automated copy of the form in their email upon submission.
• If there are questions about the request, contact may be made by phone or email.
• Notification will be communicated when the facility request has been approved.
• Once the facility request has been approved, you may complete the St. Mark Communication Request Form for marketing and promotions if needed.
